MESSAGING PROGRAM TERMS

NOTICE: The Amgen Cares mobile program, a subset of the XGEVA® Cares program, is not intended to be a source of medical advice or care. Please contact your healthcare provider if you have any questions about your medical condition, diagnosis, treatment, or care.

Once enrolled in the Amgen Cares mobile program, you can sign up for helpful support communications.

Enrollment and Getting Started
Within the support communication program registration section on www.XGEVA.com/sign-up, specific to the text message option, you will be asked to provide the following information:
  • Cell Phone Number: This is the number where you would like the support communications to be delivered
  • Time Zone: This is important to provide so the text messages are delivered at an appropriate time of day

Once you enroll in the Amgen Cares mobile program and choose to receive support communications, you will receive a welcome text message from Amgen Inc. confirming your desire to enroll in the Amgen Cares mobile program support communications. You will need to reply YES to confirm enrollment. Should you not confirm enrollment, you will not be enrolled in receiving text support communications for the Amgen Cares mobile program.

How Many Text Messages Will I Receive?
The amount of text support communications received will vary per month based on your registration preference. On average, you may receive 2 messages per month.

Opting Out
You can opt out of the Amgen Cares mobile program support communications at any time by calling 1-888-4ASSIST.

Should you have any questions about XGEVA® or the Amgen Cares mobile program, please do not reply to any text message sent to you through the support communications program, as responses are not monitored. Instead, please call 1-888-4ASSIST and a helpful Amgen Cares mobile program representative can help assist you.

The Amgen Cares mobile program is a Short Message Service (SMS) which runs on short code owned by Amgen Inc. Amgen will not charge you to use this Service; however, your Wireless Service Provider may charge for sending and/or receiving messages and for airtime.

Amgen Inc. (“Amgen”) reserves the right, in its sole discretion, to change, modify, add, or remove these Mobile Terms of Use (“Mobile Terms”) at any time. Amgen may in its discretion change or suspend the Service (defined below) at any time. If you are dissatisfied with the Service or the content received through the Service, your sole remedy is to discontinue use of the Service.

By using the Service and accepting these terms, you also agree to Amgen’s standard Terms of Use, incorporated herein by reference. In the event of a conflict between the standard Terms of Use and these Mobile Terms, these Mobile Terms shall prevail.

To Opt-In to the Amgen Cares mobile program or Receive Subscription Messages:
Visit www.XGEVA.com/sign-up or call 1-888-4ASSIST to enroll. Provide the required information including your mobile phone number. You will receive an opt-in request from Amgen Inc. to your mobile phone number. Follow the texting prompts to complete your subscription.

To Stop (Opt-Out of) the Service:
You can cancel receipt of all SMS messages from Amgen Inc. by replying to any Service message or sending STOP to Amgen Inc.

Example Stop Message:
